The pay range indicates the minimum and maximum compensation for this role. Salaries will be determined based on factors such as market data, education, experience, and internal equity.
Provides individualized assistance, support, and comprehensive coordination of care for patients and their families/caregivers. Aims to eliminate barriers to timely care, facilitate system flow, enhance interactions with the care team, increase satisfaction, and serve as the primary contact point.
This is a full-time position, primarily M-F days, with evenings and weekends as needed.
Joining the Hospital at Home team offers a unique opportunity to be part of developing and expanding an innovative care model, with an anticipated launch date of July 1, 2025. The Nurse Navigator will guide patients through the continuum of care, utilizing screening tools to identify suitable candidates, educating patients and families about H@H, and ensuring smooth transitions to home care.
Requirements include strong communication skills, independence, proactive problem-solving abilities, BSN degree, and at least 3 years of acute care, ICU, or ED experience. RN navigation or care coordination experience is preferred. A valid NYS driver’s license and insured personal vehicle are required.
