Job summary

Hackensack Meridian Health is seeking a Per Diem Hospital at Home Nurse Practitioner/Advanced Practice Nurse to provide patient-centered, coordinated care. The APN will collaborate with the practice team, assess, plan, and implement care for individuals with health and safety needs in a hospital-at-home setting.

The role emphasizes leadership, clinical practice, consultation, coaching, and research, with weekend and holiday work as needed and no on-call requirements.

Qualifications

• Graduate of an NLN/AACN accredited nursing program.
• Master's or Doctorate degree from an accredited Nurse Practitioner program.
• Maintains clinical competencies through continuing education and clinical practice.
• Experience in a relevant patient care setting (or as regulations dictate).
• National Provider Identifier (NPI).
• Prescriptive authority through CDS and DEA registration according to New Jersey state law.
• Collaborative agreement with physicians on staff.
• Excellent written and verbal communication skills.
• Proficient computer skills including Microsoft Office and/or Google Suite.

Responsibilities

• Performs and documents accurate, complete, and relevant history and physical assessments appropriate for the patient's age, gender, and clinical problem.
• Orders, performs, and interprets laboratory tests and diagnostic studies in collaboration with physicians and consultants as appropriate.
• Manages general medical and surgical conditions based on etiologies, risk factors, and pharmacologic considerations.
• Prescribes medication safely and effectively in accordance with best practices and NJ regulations.
• Promotes APN practice recognition through conferences, publications, and presentations.
• Provides patient- and family-centered care, with focus on health promotion and disease treatment.
• Adheres to nursing protocols, standards of care, and quality indicators.
