Schedule Details/Additional Information: This is a full-time hybrid position supporting the Hospital at Home program at Navicent Medical Center. The schedule consists of a mixed rotation of day shifts, including weekends, swing shifts, holidays, and weekend coverage based on operational needs. The role combines virtual patient care with in-home visits throughout the service area.
Pay Range: $51.05 - $76.60

Advocate Health is seeking an Acute Care Nurse Practitioner or Physician Assistant to join the Hospital at Home team at Navicent Medical Center. This full-time hybrid opportunity supports the expansion of the Hospital at Home program to Navicent Medical Center. The Hospital at Home APP provides specialized, patient-centered care to acute and chronically ill adult patients through a combination of virtual and in-home services. In collaboration with physicians and a multidisciplinary healthcare team, the APP delivers comprehensive inpatient-level care while helping expand an innovative care model designed to improve quality, access, and patient outcomes.
Work requires walking, standing, sitting, lifting, reaching, bending, stooping, pushing, and pulling. Must be able to lift and support up to 35 pounds while handling patients, medical equipment, and supplies. Must speak English fluently and possess the ability to react appropriately in stressful situations. Requires the ability to use sound judgment, communicate effectively, and work independently in patients’ homes and virtual care environments.
Demonstrates strong critical thinking, sound clinical judgment, excellent interpersonal skills, and the ability to prioritize, multitask, and concentrate in a fast-paced environment.
