Enjoy working in a fast-paced, challenging environment? If yes, join our team! The Hospital At Home program offers a dynamic care model where suitable patients are transferred to a virtual unit to complete their hospitalization, similar to a regular inpatient unit but set in the patient's home. The program supplies medical equipment, meals, and diagnostics in the home setting. The Field RN provides hands-on care and collaborates closely with a team of clinicians, including Command Center RNs, MDs, and therapists such as PT and OT. This role involves working dynamically with virtual colleagues to deliver clinical care.
Hospital at Home manages patients meeting inpatient criteria in their homes, leading to better outcomes, higher satisfaction, fewer complications, and shorter stays. Staff report higher job satisfaction caring for these patients virtually and in-home. The acuity level is comparable to a standard medical floor. While not suitable for all, this model is appropriate for patients who meet specific criteria and are deemed safe for home hospitalization.
Ideal candidates will possess excellent clinical and communication skills, be comfortable with technology, and adaptable as daily needs change. Join a supportive team! The role requires obtaining nursing licenses for supported states.
