Job summary

Advocate Health seeks a Hospital at Home Clinical Pharmacist to deliver comprehensive pharmacy services for patients receiving acute hospital-level care at home. You will manage therapy, perform clinical review, reconciliation, and coordinate safe delivery while collaborating with physicians, nurses, and mobile teams.

This role supports expansion of Central Pharmacy services and helps build scalable workflows for medication management in the home setting, including oversight of hospital at home,

Qualifications

• Licensed pharmacist and eligibility for practice in state(s)
• Immunization and non-vaccine injectable certification required
• ACPE-accredited PharmD or NABP FPGEC certificate or equivalent

Responsibilities

• Oversee pharmaceutical care and design therapy regimens
• Counsel patients and caregivers on therapy, safety and adherence
• Review medication orders and participate in emergencies
• Support formulary adherence and medication-use processes
• Precept students and mentor staff
