Job summary

Advocate Health Care seeks a Hospital at Home Clinical Pharmacist to provide comprehensive pharmacy services for patients receiving acute hospital-level care in the home. The pharmacist will manage medication therapy, conduct clinical reviews, reconcile medications, and coordinate delivery with mobile care teams.

The role emphasizes collaboration with physicians, nurses, and pharmacy operations to optimize therapy and ensure safe, scalable workflows in the home setting.

Qualifications

Hospital experience or clinical pharmacy background in hospital settings preferred.
Licensure as pharmacist in state of practice or eligibility for reciprocity required.
Bachelor’s or Doctorate degree in pharmacy from ACPE-accredited school; NABP FPGEC acceptable.

Responsibilities

Oversees pharmacotherapy management and medication-related clinical review.
Counsels patients and caregivers on therapy, adverse effects, compliance, and handling.
Reviews medication orders and participates in medical emergencies when needed.
Supports safe medication delivery, formulary adherence, and medication-use workflow improvements.
