Job summary

Hallmark Healthcare of Carlinville is seeking a fully remote Hospital Admissions Liaison to support weekend admissions for long-term care and skilled nursing facilities. You will review referrals from hospitals across the St.

Hospital Admissions Liaison

Fully Remote | Full-Time | Friday, Saturday & Sunday

Three 12-Hour Shifts | $23–$25/hour

We are hiring a fully remote Hospital Admissions Liaison to support weekend admissions for our portfolio of long-term care and skilled nursing facilities. You will work alongside our other Hospital Liaisons reviewing and coordinating referrals from hospitals throughout the St. Louis region, Springfield area, and surrounding communities.

Qualifications

• Long-term care or skilled nursing admissions experience required
• Must be able to confidently read and interpret hospital referrals, medical records, and clinical documentation
• Nursing or clinical background is recommended, but not required
• Strong computer skills and ability to navigate multiple referral systems
• Organized, detail-oriented, and able to work independently
• Must reside in IL or MO

Responsibilities
