Job summary

Suncrest Hospice is seeking a full-time weekend RN to cover the Friday 4:30 pm to Monday 8:00 am shift in Ohio. The RN will provide hospice care to patients in home settings, facilities, and hospitals, delivering admissions and patient visits with strong clinical and communication skills.

The role requires current RN licensure in good standing, on-call experience is preferred, and the ability to work with a multidisciplinary team while managing visits, documentation, and triage.

Qualifications

• Current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
• Admissions and On-call experience preferred.
• Works well within a team environment with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Collaborates with patients one-on-one while receiving support from the patient care team.
• Capable of independent work with calls, meetings, and triage as needed.
• Strong time management to schedule visits and complete documentation.
• Willing to learn and use an electronic medical records system.
• Must be willing to travel across the service area with reliable transportation.

Suncrest Hospice is seeking a full-time weekend RN. This position will work Friday starting at 4:30 pm until Monday at 8:00 am.

The Registered Nurse (RN) will be responsible for providing care and services for a group of hospice patients, visiting patients where they reside, which could include private homes, assisted living facilities and hospitals. As a RN, you will complete hospice admissions and patient visits as needed. Applicants must have great clinical and communication skills. The RN is responsible for answering calls from patients, families and customers. The RN will make nursing visits for symptom management, deaths, and admissions as needed. This RN must also be passionate about hospice care and be a great advocate for both hospice services and Suncrest Hospice.

Qualifications

• Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice with one year of experience under license
• Admissions and On-call experience preferred
• Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Work closely with patients on a 1-1 level but get strong support from a patient care team.
• Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
• Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)
• Ability to learn an electronic medical records system.
• Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance, to be able to travel across the service area
