Job summary

Guardian Angel Hospice is seeking a Weekend Option Nurse to deliver specialized end-of-life care on weekend shifts. You will work closely with physicians, families, and the hospice team to ensure comfort, safety and symptom control in alignment with approved care plans.

This role emphasizes compassionate communication, on-site assessments, and collaboration with the care team to uphold patient preferences and high-quality care in the final stage of life.

Qualifications

• Licensed RN with active Indiana license.
• Experience in end-of-life care and hospice settings.
• Ability to communicate clearly with patients, families and providers.

Responsibilities

• Provide direct care during weekend shifts in alignment with care plan.
• Re-evaluate nursing needs and adjust as directed.
• Administer and monitor pain control, symptom management, and infusions.
• Coordinate with physicians, family and home health team on care changes.
• Educate patients/families on Advance Directives and care options.
• Maintain policies and safety standards in hospice care.

The Weekend Option Nurse plays a key role in providing specialized nursing care that aligns with the physician-directed and team-developed plan of care. With a focus on patient comfort, safety, and clinical excellence, this nurse supports both patients and their families during critical weekend hours.

• Evaluates and routinely re-evaluates the nursing needs of the patient
• Provides direct care as needed during the weekend
• Demonstrates competency in all skills required to provide world-class end-of-life care, including, but not limited to, pain management, symptom control, infusion therapy, central line care and maintenance, wound care, and aseptic technique
• Responds to medical or emotional crisis and/or death of a patient, and makes on-site visits.
• Advocates for the patient and the family to ensure the patient's preferences are honored
• Establishes and maintains positive relationships with those served
• Assists with the education of the patient/family on Advance Directives
• Supervises and teaches the home health aide
• Initiates communication with attending physicians and other community resources to report changes as needed
• Demonstrates familiarity with policies of the agency and rules, regulations of community, state, and federal bodies, which relate to the delivery of hospice services

Requirements:

• Must have reliable transportation and a valid driver's license
