Guardian Angel Hospice in Kokomo, Indiana seeks an After-Hours On-Call Nurse to provide compassionate, physician-directed end-of-life care. You’ll support patients and families during critical after-hours periods with a focus on comfort, safety, and clinical excellence.

Formación

• Must hold a valid RN license.
• Experience in hospice or end-of-life care preferred.
• Ability to work after-hours and on-call shifts.

Responsabilidades

• Evaluates and reassesses patient nursing needs during on-call hours.
• Provides direct nursing care during on-call periods as needed.
• Demonstrates competency in end-of-life care, including pain management and symptom control.
• Responds to medical or emotional crises and conducts on-site visits when required.
• Advocates for patient and family preferences and coordinates with attending physicians.
• Educates patient/family on Advance Directives and care planning.
• Supervises and teaches the home health aide as needed.
• Communicates changes to attending physicians and community resources.
• Adheres to agency policies and applicable regulations.

The After-Hours On-Call Nurse plays a key role in providing specialized nursing care that aligns with the physician-directed and team-developed plan of care. With a focus on patient comfort, safety, and clinical excellence, this nurse supports both patients and their families during critical after-hours periods.

This position follows a 7 days on, 7 days off schedule, providing on-call nursing support outside of regular business hours.

Responsibilities:

• Evaluates and routinely re-evaluates the nursing needs of the patient
• Provides direct care as needed during on-call hours
• Demonstrates competency in all skills required to provide world-class end-of-life care, including, but not limited to, pain management, symptom control, infusion therapy, central line care and maintenance, wound care, and aseptic technique
• Responds to medical or emotional crises and/or death of a patient, and makes on-site visits as needed
• Advocates for the patient and the family to ensure the patient's preferences are honored
• Establishes and maintains positive relationships with those served
• Assists with the education of the patient/family on Advance Directives
• Supervises and teaches the home health aide
• Initiates communication with attending physicians and other community resources to report changes as needed
• Demonstrates familiarity with policies of the agency and rules and regulations of community, state, and federal bodies, which relate to the delivery of hospice services

Requirements:

• Must have reliable transportation and a valid driver's license
