Job summary

Tri-Cities Chaplaincy is seeking an experienced Hospice Visit RN to provide compassionate, patient-centered care during evenings, nights, weekends, and holidays. You will respond to urgent medical needs, manage symptoms, coordinate with the interdisciplinary team, and ensure comfort for patients at the end of life.

The role requires a WA RN license, 2+ years of nursing experience (hospice/oncology preferred), strong communication skills, and proficiency with EMR systems.

Qualifications

• WA State registered nurse license required.
• At least two years nursing experience; hospice/palliative/oncology preferred.
• Ability to work independently and stay calm under pressure.
• Availability evenings, nights, weekends, and holidays.
• Excellent communication with patients, families, and team.
• EMR experience and basic computer skills.
• Safety-sensitive; pre-employment drug screen incl cannabis in WA.
• CPR/BLS certification.
• WA Food Worker Card required.

Responsibilities

• Assess, plan, implement, and evaluate nursing care for hospice patients on duty.
• Respond to urgent calls from patients, caregivers, and families.
• Manage end-of-life symptoms including pain, respiratory distress, wounds, agitation.
• Provide direct nursing care and wound care.
• Educate and support caregivers for at-home symptom management.
• Perform on-site home or facility visits as needed.
• Communicate patient status to the interdisciplinary team via EHR.
• Coordinate with physicians and supervisors to adjust care plans.
• Offer emotional support to patients and families during crises.
• Document all patient interactions accurately in the EHR.
• Participate in team meetings and quality improvement activities.
• Support admissions, information meetings, and in-home hospice care as assigned.
• Perform other duties as assigned.
