Job summary

AccentCare Inc. in the Greater Denver area is seeking a Hospice Visit Physician to evaluate patients for pain and symptom management and determine hospice eligibility, including admissions and recertifications.

You will counsel patients and families on care options and advance care planning, order tests, initiate treatment, and conduct face-to-face visits as needed. Serve as Attending Physician when appropriate, coordinating with interdisciplinary teams for continuity, and ensuring timely

Qualifications

• Maintains active medical and DEA licenses in the state of Colorado.
• Hospice or hospice visit experience preferred.

Responsibilities

• Evaluate patients for pain, symptom management, and hospice eligibility (admissions and recertifications).
• Counsel patients and families on care options and advance care planning.
• Order tests, initiate treatment, and conduct face-to-face visits as needed.
• Serve as Attending Physician when appropriate.
• Coordinate care with interdisciplinary teams and communicate with attending physicians to ensure continuity.
• Manage patient medications.
• Participate in on-call rotation and provide 24/7 availability when assigned.
• Complete timely and compliant clinical documentation.
