Job summary

AccentCare Inc. is seeking a Hospice Visit Physician to support the Greater Minneapolis/St. Paul area.

This PRN role offers per-visit compensation and requires an active Minnesota medical license and a DEA license. Hospice visit experience is preferred and candidates should be committed to compassionate, patient-centered end-of-life care. You will evaluate patients for pain and symptom management, counsel families on care options, and coordinate with interdisciplinary teams.

Qualifications

• Must maintain an active medical license in Minnesota.
• Must hold a valid DEA license.
• Experience in hospice visits is preferred.

Be the Best Hospice Visit Physician You Can Be

If you meet these qualifications, we would love to meet you:

• Maintains active medical and DEA license in the state of Minnesota
• (Preferred) Hospice or hospice visit experience

Job Responsibilities

• Evaluate patients for pain, symptom management, and hospice eligibility (admissions and recertifications).
• Counsel patients and families on care options and advance care planning.
• Order tests, initiate treatment, and conduct face-to-face visits as needed.
• Serve as Attending Physician when appropriate.
• Coordinate care with interdisciplinary teams and communicate with attending physicians to ensure continuity.
• Manage patient medications.
• Participate in on-call rotation and provide 24/7 availability when assigned.
• Complete timely and compliant clinical documentation.
