Agrace, a national hospice and palliative care leader, is seeking a part-time Triage RN in Madison, WI. The role focuses on telephone triage, patient assessment, and care coordination with the interdisciplinary team to ensure patient- and family-centered hospice services.
The role operates 0.7 FTE (28 hours/week), Monday through Friday, 12:00 pm–8:30 pm, with every-other weekend and some holiday rotations, offering a meaningful, patient-centered nursing opportunity within a supportive team.

The Triage RN is responsible for coordinating direct and indirect patient care that is based on nursing process. The Triage RN will assess each call and determine if a visit is needed or if assistance can be handled over the telephone. The Triage RN maintains accountability for nursing practice, patient outcomes, nursing assessments, interventions, evaluations documentation and coordination of care. This position ensures hospice services are patient and family centered and consistent with philosophy, goals, standards, regulations and policies of the program and Agrace.
