Job summary

Agrace in Wisconsin is seeking an experienced RN – New Market to provide patient- and family-centered hospice and palliative care. You will evaluate potential patients, admit to home-care programs in new markets, and coordinate care with the IDT to meet care goals.

The role requires clinical excellence, strong communication, and the ability to develop care plans, educate families, and document care in the EHR. Some travel and on-call rotations are involved, with day shifts and a 40-hour week.

Qualifications

  • Current, unrestricted Registered Nursing License in the state of employment.
  • One year of nursing experience, preferred.
  • Valid driver’s license, auto insurance, and access to a reliable vehicle.
  • Knowledge of hospice and end-of-life processes.
  • Professional boundaries with patients, families, and co-workers.
  • Ability to develop, maintain, and follow care plans.
  • Basic Microsoft Office skills; Epic preferred.
  • Confidentiality regarding patients, families, caregivers, staff, donors, and organizational information.

How You’ll Make a Difference (Essential Functions)

The RN – New Market provides patient‑ and family‑centered care in new geographic markets; they establish, deliver, and ensure care consistent with Agrace philosophy, goals, standards, regulations, and policies. The role evaluates potential patients for hospice and palliative care, admits patients to Agrace’s home‑care program in new markets, and ensures patient and family satisfaction through assessments, triage, care‑plan development, interventions, communication, and documentation. Percentages of time for each essential function may vary based on needs.

Key Responsibilities
Patient and Family Assessments and Evaluations
  • Review medical records to identify hospice and palliative care eligibility, physician orders, and to verify diagnosis.
  • Determine whether the patient meets admission criteria in collaboration with primary care, specialty, and Agrace physicians.
  • Collaborate with IDT and primary care provider to create HES for hospice eligibility determination.
  • Assess patient’s physical, pain, psychosocial, emotional, spiritual, environmental, fall, medication safety, and educational needs.
  • Collect data from the patient, family, significant others, and health‑care professionals about immediate and longer‑term goals.
  • Serve as consultant to patients and families about end‑of‑life issues and Agrace programs’ philosophies and services.
  • Interpret and document assessment data collected during the nursing interview, physical assessment, and other data.
  • Serve as resource and consultant for hospital/clinic staff, physicians, nurses, social workers, chaplains on pain management, symptom control, end‑of‑life issues, hospice philosophies and services.
  • Document and report significant changes in condition or unusual incidents to the appropriate IDT member.
  • Collaborate with manager regarding all patients deemed ineligible for hospice services.
  • Pursue relationships and develop partnerships with referral sources to promote Agrace services.
Patient Care
  • Assess patients and families’ physical, pain, psychosocial, emotional, spiritual, environmental, and educational needs as outlined by the plan of care.
  • Respond to patient and family needs or changes in condition requiring acute intervention.
  • Provide interventions, treatments, and pain management to meet needs; evaluate effectiveness and adjust as necessary.
  • Educate patient and family on pain, symptom management, end of life; evaluate effectiveness and adjust as necessary.
Plan of Care
  • Formulate, implement, and revise individualized POC in collaboration with IDT, patient, and family.
  • Develop measurable outcome measures and establish priorities in the planning and provision of care.
  • Obtain medication and other orders.
  • Order durable medical equipment.
  • Read, understand, and follow the plan of care; document interventions, outcomes, and changes in patient condition in the electronic health record.
  • Delegate visits to appropriate disciplines and manage visit sets.
  • Oversee work of Certified Nursing Assistants (CNA) and Licensed Practical Nurses (LPN).
Miscellaneous
  • Participate in meetings to plan and evaluate patient/family services.
  • Precept new staff and students.
  • Participate in education and outreach.
