Qualifications

• Minimum of two years’ experience as a registered nurse in hospice or related healthcare setting.
• Current state license as a registered nurse (RN).
• Valid and current CPR certification.

Responsibilities

• Coordinate and facilitate smooth patient transitions into hospice services.
• Educate patients and families regarding hospice philosophy.
• Collaborate with physicians and case managers to ensure continuity of care.
