Job summary

Enhabit Home Health & Hospice is seeking a compassionate, licensed nurse to support patient transitions and coordinate home health services from the branch level. You will liaise with patients, families, physicians and hospitals to ensure smooth handoffs and high-quality care.

The role requires one year of home health experience, regulatory knowledge, and basic tech skills including a mobile device. A valid nursing license in the applicable state is essential.

Qualifications

• RN license in the state where you practice.
• At least 1 year of home health experience.
• Knowledge of federal, state and local hospice regulations.
• Basic technology skills including use of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and professional contacts.
• Develop patient-centered transition plans aligned with clinical guidelines and metrics.
• Educate patients and families about provided services.

Responsibilities

Represent the branch in external transitional care activities, and in professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and similar health groups and institutions, to apprise them of the availability of Enhabit Home Health & Hospice services. Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency. Serve as a public awareness representative and will be responsible for education related to provided services.

Qualifications

Education and experience, essential

• Must be a graduate of an approved school of clinical education
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of a hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education and experience, preferred

• Demonstrated experience with and knowledge of hospice is preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
