Job summary

Enhabit Home Health & Hospice is recruiting qualified clinicians to support patient-centered transitions and coordination across patients, families, and care teams. The role emphasizes adherence to clinical guidelines and regulatory standards while improving care quality and efficiency.

The position emphasizes professional collaboration, education, and public outreach to promote available home health services and ensure seamless transitions for patients across care settings.

Qualifications

• Must have completed education and licensure as described.
• Must comply with applicable hospice regulations and standards.
• Experience in home health or hospice settings is advantageous.

Responsibilities

• Represent the branch in external transitional care activities with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and similar health groups to inform them of Enhabit Home Health & Hospice services.
• Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency.
• Serve as a public awareness representative and provide education related to provided services.

Education

Graduate of an approved school of clinical education

Licensed in the state where they currently practice

At least one year experience in home care

Demonstrated experience and understanding of federal, state, and local laws governing hospice

Basic demonstrated technology skills including operation of a mobile device

Experience with hospice is preferred

Represent the branch in external transitional care activities, and in professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and similar health groups and institutions, to apprise them of the availability of Enhabit Home Health & Hospice services. Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency. Serve as a public awareness representative and will be responsible foreducation related to provided services.

Qualifications

Education and experience, essential

• Must be a graduate of an approved school of clinical education
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of a hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education and experience, preferred

• Demonstrated experience with and knowledge of hospice is preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
