Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year of experience in home care.
• Must have demonstrated knowledge of federal, state, and local laws and regulatory guidelines governing hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and in professional contacts with patients, families, physicians, hospitals, facilities, and health groups to apprise them of Enhabit services.
• Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency.
• Serve as a public awareness representative and provide education related to provided services.

Integrate clinical guidelines, protocols, and other metrics to develop patient‑centered transition plans that promote quality and efficiency. Serve as a public awareness representative and will be responsible for education related to provided services.

Education and Experience, Preferred

• Demonstrated experience with and knowledge of hospice is preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
