Job summary

Buckeye Hospice and Palliative Care Services is seeking a compassionate full-time hospice aide to serve in the Bowling Green, OH area and surrounding communities.

Role involves providing personal care under RN supervision, documenting care, and participating in team meetings. Must have valid Ohio Nurse Aide registry in good standing and at least 1 year of CNA/hospice experience; driver’s license required. Mileage is reimbursed.

Qualifications

• Must be listed in good standing on the Ohio State Nurse Aide registry (STNA).
• At least 1 year experience as CNA, hospice aide or home health aide.
• Must be 18 years or older.
• Must have a valid driver’s license with insured vehicle.
• Ability to communicate well, verbally and in writing.

Responsibilities

• Provide personal care under the direction of a registered nurse.
• Perform personal care (bathing, dressing, toileting, feeding).
• Document care per agency policy.
• Report deviations from the Plan of Care to the Case Manager.
• Use Durable Medical Equipment safely.
• Take vital signs per Hospice Aide Plan.
• Participate in team conferences and IDG meetings.
• Observe skin condition and report changes to Case Manager.
• Assist with ambulation, transfers, ROM as documented.
• Provide respite for family/caregiver on-site.
• Participate in QAPI activities.
