Job summary

Interim HealthCare Hospice & Palliative Care is seeking a Hospice Spiritual Care Coordinator-Chaplain to support a full range of patient services and bring comfort and dignity to clients. You will ensure timely spiritual services and coordinate with clergy or counselors, while maintaining records for quality and program development.

Experience in grief counseling and hospice is preferred; graduate education in theology is preferred.

Qualifications

• Experience with grief counseling and working with death and dying, terminal individuals and their families or caregivers.
• Hospice experience preferred.
• Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree) preferred.

Responsibilities

• Ensures that appropriate spiritual services are provided in a timely manner by either providing such to the patient/family or coordinating the provision of services with community clergy or spiritual counselors.
• Provides direct spiritual services to patients/families according to their belief system and practice.
• Maintains records of spiritual services utilization and related activities for quality assessment and program development.
• Acts as the hospice spiritual services community liaison by developing community contacts and offering hospice education.

As a HospiceSpiritual Care Coordinator-Chaplain, you will:

• Ensures that appropriate spiritual services are provided in a timely manner by either providing such to the patient/family or coordinating the provision of services with community clergy or spiritual counselors.
• Provides direct spiritual services to patients/families according to their belief system and practice.
• Maintains records of spiritual services utilization and related activities as directed for quality assessment and performance improvement (QAPI), hospice program development, and policies and procedures review and revision.
• Acts as the hospice spiritual services community liaison by developing community contacts and offering hospice education to a variety of clergy, counselors and congregations.
