Job summary

Angels Care Hospice is seeking a Hospice Chaplain to provide spiritual care to patients, families, and caregivers, coordinating with other spiritual counselors as needed. The role includes assessment, care planning, direct spiritual care, and collaboration with the interdisciplinary team.

The chaplain will recruit community chaplains and volunteers, document services, participate in quality improvement, and may officiate funeral or memorial services as requested.

Qualifications

• Graduate of accredited seminary or school of theology (BD, MDiv, or equivalent theological degree).

Responsibilities

• Assesses spiritual needs of patients and families and provides appropriate services in a timely manner.
• Develops individualized care plans by participating in comprehensive spiritual assessments.
• Provides direct spiritual care to patients and families/caregivers.
• Serves as liaison and supports community chaplains and spiritual counselors.
• Maintains records of spiritual care services and related activities for quality assurance and review.
• Attends patient care conferences and interdisciplinary group meetings as a member of the team.
• Provides consultation, education, and support to the interdisciplinary group on spiritual care.
• Recruits community chaplains, spiritual counselors, and volunteers to meet patient needs.
• Provides funeral or memorial services for patients as requested.
• Plans memorial services for personnel and community clergy/spiritual counselors.
• Assists in supervision of spiritual care volunteers assigned to patients/families.
• Participates in quality assessment and performance improvement activities.
• Accepts and performs other related duties as required.
