Ascend Hospice & Palliative Care MO in Lee's Summit, MO is seeking a dedicated Social Worker to join our hospice team. The role focuses on psychosocial assessment, counseling, and coordinating care for patients and families facing end-of-life challenges.

Responsibilities include emotional support, crisis intervention, and collaboration with community resources. A Master’s in Social Work, 1 year hospice-related experience, an active LSW license, and willingness to travel are required.

Qualifications

• Master's degree in Social Work required.
• Minimum 1 year of Medical Social Work experience, Hospice preferred.
• Must have an LSW certificate.
• Understanding of hospice philosophy for families in crisis and end of life issues.
• Ability to multitask and prioritize workload.
• Must be willing to travel.
• Strong verbal and written communication skills.

Responsibilities

• Assess emotional factors related to terminal illness.
• Assess environmental resources and obstacles to safety.
• Provide emotional support, crisis intervention and counseling as indicated.
• Coordinate with community agencies to facilitate referrals and interagency cooperation.
• Evaluate patient/family response to psychosocial interventions.
• Consider cultural diversity in communication, space, roles, and traditions.
• Perform initial psychosocial assessments and assess risk of suicide or abuse.
