About the Role

We are seeking an experienced, full-time Hospice Social Worker for our Philadelphia, PA region.

Responsibilities
  1. Conduct psychosocial assessments within 5 days of admission, covering emotional, social, financial, and environmental resources, and develop appropriate care goals.
  2. Participate in interdisciplinary group (IDG) meetings and develop patient care plans.
  3. Observe, record, and report changes in patients’ emotional and social well-being related to their illness and treatment response.
  4. Maintain and submit clinical records, including evaluations, care plans, and daily notes.
  5. Evaluate the effectiveness of social work interventions on patients and families.
  6. Coordinate weekly visits with case managers to facilitate care.
  7. Participate in IDG and QAPI meetings and activities.
  8. Support patients and families who decline services, monitoring their evolving needs.
  9. Assist with discharge planning as needed.
  10. Strive for ongoing professional development through education, workshops, conferences, and research.
  11. Assess coping abilities of patients and families regarding end-of-life issues.
  12. Consult with health team members to address social, emotional, and environmental factors affecting health.
  13. Assist patients in utilizing family and community resources.
Qualifications
  1. Bachelor’s degree in Social Work (BSW).
  2. At least one year of post-bachelor’s social work experience in healthcare.
  3. Hospice experience preferred.
  4. Ability to handle confidential information reliably.
  5. Valid driver’s license with insured vehicle in good condition.
  6. Strong verbal and written communication skills.
