Job summary

Caretenders, part of LHC Group, is seeking a Social Worker - Hospice Per Diem in Massachusetts. You will provide social work services under physician supervision to support patients and families with emotional and social factors related to health, contributing to an integrated Plan of Care.

The role includes bereavement coordination, collaboration with the care team, and utilization of community resources. Requires a MSW, LICSW where applicable, and a valid driver's license.

Qualifications

• Master's Degree from a school of Social Work accredited by the Council of Social Work.
• 1+ years of social work experience in a health care setting.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Assists patient, family, physician and care team to understand social factors affecting health status.
• Contributes to development of a comprehensive Plan of Care.
• Instructs team on community resources for patient support.
• Able to function as Bereavement Coordinator and plan bereavement services as needed.
