Job summary

Caretenders, a part of LHC Group, seeks a Medical Social Worker in Needham, MA to provide hospital and home-based social work under physician direction and an interdisciplinary team.

The role focuses on addressing social and emotional factors affecting health, developing and updating a Plan of Care, and guiding patients and families to community resources. Bereavement coordination may be required.

Qualifications

• Master's Degree in Social Work from a CSWE-accredited program.
• Current CPR certification.
• Licensed Social Worker in the state of residence.
• Current Driver's License, vehicle insurance, and access to dependable transportation.
• 1+ years of social work experience in a health care setting.

Responsibilities

• Assist the patient, family, physician and health care team to understand significant personal, emotional, environmental and social factors related to the patient's health status.
• Contribute to the development of a comprehensive, integrated Plan of Care for patients on a daily basis.
• Instruct health care team members on community resources available to assist patients.
• Serve as Bereavement Coordinator and supervise bereavement services as needed, with a clear plan of care.
