Qualifications

• Social worker responsible for implementing care standards.

Responsibilities

• Assesses the psychosocial status of patients and families related to the patient’s terminal illness.
• Evaluates residence safety and plans when this is unsafe or a different plan is required.
• Performs social evaluations, including family dynamics, caregiver abilities, communication patterns, suicide risk, neglect or abuse and intervention plans.
• Counsels patients, families & caregivers on stress and coping difficulties.
• Provides crisis intervention when necessary.
• Educates interdisciplinary staff on cultural needs of the patient and family.
• Maintains clinical records on all patients referred to social work; prepares clinical and progress notes.
• Provides information and referral services for patients, families, & caregivers regarding practical and environmental needs.
• Serves as liaison between patients, families, caregivers, and community agencies.
• Maintains collaborative relationships with organization personnel to support patient care.
• Develops contracts with public and private agencies as resources for patients and personnel.
• Actively participates in quality assessment performance improvement teams and activities.
