Angels Care Hospice in Wichita Falls, TX is seeking a social worker to implement standards of care for medical social work services within our hospice program. The role emphasizes compassionate support for patients and families and collaboration with the care team.

The position involves assessing psychosocial needs, planning with caregivers, crisis intervention, and maintaining records to ensure quality patient care.

Formación

• MSW or related licensure preferred.
• Experience with terminal illness or palliative care is a plus.
• Strong interpersonal and documentation skills.

Responsabilidades

• Assess patients' and families' psychosocial status related to terminal illness.
• Evaluate residence safety and plan alternatives when needed.
• Conduct social evaluations including family dynamics and caregiver abilities.
• Counsel patients and families on coping and stress.
• Provide crisis intervention when necessary.
• Educate the care team on cultural needs of patients and families.
• Maintain clinical records and progress notes for referred patients.
• Coordinate information and referrals with patients, families, and community resources.
• Serve as liaison between patients, families, caregivers, and community agencies.
• Maintain collaborative relationships to support patient care.
• Develop contracts with agencies as resources.
• Participate in quality improvement activities.

Job Description:

The social worker contracted or employed through the organization is responsible for the implementation of standards of care for medical social work services.

Job Functions:

• Assesses the psychosocial status of patients and families/caregivers related to the patient’s terminal illness and environment.
• Provides an assessment of the patient’s identified residence and assistance when this is unsafe, and another plan is required.
• Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risks for suicide, neglect or abuse and plans intervention.
• Counsels patients, families & caregivers as needed in relationship to stress, and other identified coping difficulties.
• Crisis intervention when necessary.
• Educates interdisciplinary group on any special needs related to the culture of the patient and family.
• Maintains clinical records on all patients referred to social work; prepares clinical and progress notes.
• Provides information and referral services for organization patients, families, & caregivers regarding practical and environmental needs.
• Serves as liaison between patients, families, caregivers, and community agencies.
• Maintains collaborative relationships with organization personnel to support patient care.
• Develops contracts with public and private agencies as resources for patients and personnel.
• Actively participates in quality assessment performance improvement teams and activities.
