Job summary

Heart to Heart Hospice is seeking a Social Worker to assess hospice patients’ psychosocial, grief, financial, environmental and community needs, documenting findings in the patient care plan and coordinating with the care team.

The role provides individual and family counseling, crisis intervention, guidance on advance directives and funeral planning, and facilitates access to community resources while ensuring timely documentation and adherence to regulations.

Qualifications

• Bachelor's or Master's degree in social work from a State accredited program.
• Current State license required in accordance with State regulations.
• Current Driver's License, reliable transportation, and ongoing valid auto liability insurance.
• One year of experience as a Social Worker in a clinical care setting; hospice experience preferred.

Responsibilities

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.
• Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments that are consistent with education and competence to meet the needs of the patients.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Timely submission of all documentation.
• Performance of other duties as required.
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
