Job summary

Qualifications

• Bachelor’s or Master’s degree in social work from a state accredited program.
• Minimum of one year of experience in health care.
• Experience in hospice/home health care preferred.
• Demonstrates good verbal and written communication, and organization skills.
• Possesses and maintains current CPR certification.
• Must be a licensed driver with an insured automobile and in good working order.

Responsibilities

• Assesses the psychosocial status of patients related to illness and environment and communicates findings to the RN.
• Carries out social evaluations and plans intervention based on evaluation findings.
• Maintains clinical records on all patients referred to social work.
• Provides information and referral services for patients and families.
• Liaises between patients, families, and community agencies.
• Maintains collaborative relationships with staff to support patient care.
• Works with sales team to develop strategies to increase community involvement.
• Promotes organization’s services at community events and builds contracts with agencies.
• Participates in development of total plan of care and case conferences as required.
• Assists physicians and other team members in understanding social/emotional factors related to health problems.
• Participates in discharge planning.
