Qualifications

Responsibilities

• Performs psychosocial assessment and documents.
• Empowers patient/families to negotiate with resource providers.
• Maintains documentation timeliness, completeness, accuracy, and clarity of goals.
• Consults and collaborates with other professionals to promote quality care.
• Participates in Interdisciplinary Group (IDG) meetings and updates the Plan of Care.
