Job summary

Chapters Health System is seeking a Social Worker to provide psychosocial support to patients and families across hospice, palliative, and home health services.

The role requires an MSW (CSWE accredited) with healthcare experience; travel to off-site locations and a valid driver’s license are needed. You will coordinate care, document outcomes, and collaborate with a multidisciplinary team in Florida.

Qualifications

• Master's degree in Social Work (CSWE accredited) required.
• One year of social work/counseling internship in health care.
• Home Health requires MSW; GA LCSW if working in Georgia.
• Ability to travel to off-site locations and navigate various facilities.
• Experience with EMR/EHR systems and multidisciplinary teams.

Responsibilities

• Performs psychosocial assessment and documents findings.
• Empowers patients/families to negotiate with providers and plan care.
• Collaborates with clinicians to ensure quality, coordinated care.
• Maintains timely, complete, accurate case documentation.
• Participates in Case Conferences and IDG meetings; supports bereavement care.
