Job summary

Qualifications

• Master’s degree in Social Work (CSWE accredited) required.
• SWs hired before 2009 with other degrees are grandfathered.
• SWs hired before 4/21/2019 with BSWs are grandfathered.
• Home Health requires MSW.
• Experience in health care or hospital setting preferred.
• Experience with EMR/EHR systems.
• Ability to travel to off-site locations.
• Valid driver’s license and automobile insurance.

Responsibilities

• Performs and documents psychosocial assessment.
• Empowers patient/families to negotiate with resource providers.
• Assists other clinical staff with family dynamics and planning.
• Maintains timely, complete, accurate documentation.
• Collaborates with professionals to promote quality care.
• Performs other duties as assigned.

Skills

Master’s degree in Social Work

CSWE accredited

MSW required for Home Health

Experience in health care or hospital
