Job summary

Qualifications

• Master’s degree in Social Work required.
• Licensed Social Worker (LSW) and/or Licensed Clinical Social Worker (LCSW) credentials required.
• Hospice experience preferred; related experience in social work is beneficial.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems.
• Develop bereavement plans of care addressing family member/caregiver needs.
• Educate patients and families about learning needs, faith/culture, and advance directives; facilitate self-determined decisions.
• Document psychosocial assessments and interventions in the electronic medical record; ensure timely updates to care plans.
• Coordinate with the interdisciplinary team to optimize patient care and access to resources.
• Assist with transportation barriers, referrals for financial or community resources, and applications for programs (Medicaid, etc.).
