Job Summary

Responsible for providing psychosocial care to patients and families regarding economic, social, and emotional needs under the supervision of the Clinical Manager. Ensures compliance with State/Federal laws and regulations. Supports the Village’s mission, values, and service/hospitality (PRIDE) expectations. Completes required training.

Essential Job Functions

• Assess patient/family capacity to cope with diseases, terminal illness, and death.
• Assist with utilizing community resources.
• Evaluate insurance coverage and financial resources.
• Submit paperwork timely for medical records.
• Assist in discharge planning.
• Handle on-call social work services.
• Participate in interdisciplinary team meetings.
• Assess care needs and report changes to the Clinical Manager.
• Evaluate emotional factors related to terminal illness.
• Assist IDT members with social/mental stress recognition.
• Assess psychosocial status, suicide risk, abuse, or neglect.
• Assess environmental safety and caregiver capacity.
• Develop and revise care plans.
• Provide counseling and crisis intervention.
• Address funeral planning and advance directives.
• Consider cultural diversity needs.
• Assess bereavement and grief needs.
• Identify discharge or care level change needs.
• Participate in QAPI programs.
• Follow agency policies and regulations.
• Adhere to safety regulations.
• Maintain professional conduct and handle stress well.

Qualifications

• Ability to analyze data, advise, and guide individuals.
• Effective communication with interdisciplinary teams.
• Use of body mechanics and tools.
• Logical thinking, problem-solving, and interpreting technical instructions.
• Basic math skills for calculations.
• Proficient in reading and writing; persuasive speaking skills.

Education

• Must hold LMSW in Missouri or obtain within 3 months of hire.

Working Conditions

• Assist other departments during emergencies.

Physical and Mental Demands

• Exert up to 50 lbs force occasionally.
• Visual acuity, mobility, reaching, walking, finger dexterity, talking, and hearing required.

Supervision

Occasional supervision received; no supervision given.
