Job summary

Steel City Hospice is hiring a PRN Social Worker to support Hermitage and surrounding counties. You will perform psychosocial assessments, develop plans of care with patients and families, and coordinate care across the hospice team.

This role offers flexible hours, a supportive culture, mileage reimbursement, and resources to assist with SSI/Medicaid applications and connections to community agencies.

Qualifications

• Bachelor’s degree in psychology, sociology, or related field.
• Master’s degree preferred.

Responsibilities

• Performs psychosocial assessment of patients and families/caregivers and updates plan of care.
• Provides exceptional service to patients, families, referral sources, co-workers and other customers.
• Counsels patient and family/caregivers and staff on stress and coping difficulties.
• Provides accurate and timely documentation of patient services reflecting the plan of care.
• Assists with applications for SSI, Hospice, Medicaid, and Medicaid Waiver; connects to community resources.
• Liaises between patients, families/caregivers and community agencies.

What You’ll Do:

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care. Provides an assessment in the patient’s identified residence and assistance when this is not safe, and another plan is required
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
• Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
• Serves as liaison between patients and families/caregivers and community agencies

Qualifications:

• Social Work Degree
  • Pennsylvania: Bachelor’s degree in psychology, sociology, or other field related to social work; Master’s degree preferred.
• Current State Licensure in Social Work
  • Pennsylvania: LSW or LCSW, preferred
• Minimum of one year’s experience in health care or hospice setting
• Valid driver’s license and proof of insurance is required
