Job summary

Addus HomeCare is seeking a PRN Social Worker to support Hermitage and surrounding counties. You will conduct psychosocial assessments, counsel families, and help connect patients to community resources within a hospice program.

Qualified candidates have a social work degree and PA licensure or willing to pursue LSW/LCSW, with at least one year in healthcare or hospice. A valid driver's license and insurance are required. This role offers flexible hours and a compassionate team environment.

Qualifications

• Social work degree required with licensure considerations (LSW/LCSW) and one year of healthcare or hospice experience.
• Pennsylvania state licensure required or preferred (LSW/LCSW).
• Minimum one year of health care or hospice experience.
• Valid driver’s license and proof of insurance required.

Responsibilities

• Performs psychosocial assessment of patients and families in hospice and ensures identified needs are addressed in the plan of care.
• Provide assessment in patient residence and assist when safety concerns arise.
• Counsel patients, families, and staff on coping with stress.
• Document patient services accurately and timely to reflect the plan of care.
• Assist with applications for SSI, Hospice, Medicaid, and Medicaid Waivers; connect to community resources.
• Serve as liaison between patients/families and community agencies.

Education

Bachelor's degree in psychology, sociology, or related social work field

Master’s degree preferred
