Job summary

Qualifications

• Master’s degree in Social Work (MSW) required.
• One year of social work experience in a hospice setting preferred.
• Must be a licensed driver with a reliable automobile if home visits are performed.

Responsibilities

• Assess psychosocial status of patients and families related to terminal diagnosis and home care.
• Coordinate resources and referrals for practical and environmental needs.
• Collaborate with the interdisciplinary team to develop plans of care.
• Document clinical notes and maintain progress records for each patient.
• Participate in quality improvement activities and fulfill other duties as assigned.
