Job summary

Caris is seeking a Social Worker to perform psychosocial assessments within five days of hospice admission and collaborate with the interdisciplinary team to craft the Plan of Care. You will discuss the plan with the admitting RN and conduct regular visits, documenting assessments and outcomes.

The role includes insurance assistance, discharge planning, and coordination with volunteers to support patients and families.

Qualifications

  • MSW preferred (BSW required) in Tennessee and Missouri.
  • BSW required plus two years experience in Virginia.
  • Social Work Licensure required; LMSW required in South Carolina.
  • Social Work Licensure required; LMSW preferred and minimum LBSW in Georgia.
  • BSW in Social Work with ongoing supervision by an MSW, as required by Caris.
  • One year of social work experience in a health care setting.

Responsibilities

  • Conduct psychosocial assessments within five days of admission to hospice care and collaborate with the IDT to formulate the Plan of Care.
  • Be available to discuss the Plan of Care with the admitting RN on the day of admission.
  • When requested, conduct regular social work visits and document assessments, interventions and outcomes.
  • Assist patients and families with insurance needs; evaluate coverage and help obtain Medicaid or other coverage when no payer source exists.
  • Coordinate with the Volunteer Coordinator for volunteer services.
  • Participate in the IDT Meeting and update the Plan of Care at least every 14 days; act as consultant to hospice program staff.
  • Communicate changes in the patient’s level of care status to the Patient Care Coordinator.
  • Facilitate discharge planning and transfer of patients when hospice care is no longer requested or appropriate.
  • Participate in quality improvement processes and evaluation of social services.
  • Identify psychosocial problems, set personalized, measurable goals, and document in the Electronic Medical Record.
  • Demonstrate knowledge of the Hospice Benefit and convey that knowledge to patients and families.
  • Conduct after-death bereavement assessments and coordinate follow-up bereavement telephone calls.
  • Explain advanced directives to patients/families and document decisions with the attending physician.
  • Assist with funeral arrangements as needed.
  • Provide community outreach and education related to hospice care as planned with the Hospice Administrator and other IDT members.
  • Document psychosocial interventions accurately and timely in the Electronic Record; ensure electronic information transfers at least twice daily.
  • Deliver or supervise the delivery of social services; maintain a Social Work Community Resource Guide and assist families in accessing resources.
  • Attend staff meetings to remain current in hospice policies and procedures.
  • Communicate with team members regarding changes in patient condition and give accurate reports.
  • Report changes in the emotional, social, or financial condition of the patient or family to the attending physician and document interventions; obtain physician orders for services as necessary.
  • Be available on-call for after-hours visits, including weekday evenings, weekends, or crisis situations.
  • Review and update the plan of care as needed.
