Job summary

AdvisaCare Home Health and Hospice is seeking a full-time MSW to join our compassionate team, providing psychosocial assessments, bereavement support and care planning for patients with life-limiting illnesses.

Requirements: MSW from CSWE-accredited program; one year home health or hospice experience preferred; valid driver's license with reliable transportation. We offer 401K, medical benefits, PTO, flexible scheduling and a rewarding work environment with 24/7 staffing support.

Qualifications

MSW degree from a CSWE-accredited program.
One year of experience in Home Health Care preferred.
Hospice experience preferred.
Valid Driver’s License with reliable transportation.

Responsibilities

Performs initial psychosocial, emotional, spiritual and bereavement assessments and assists in developing care plans.
Provides care under physician direction; participates in IDT conferences and communicates psychosocial findings.
Documents patient/family visits, calls and referrals accurately in the chart.
Conducts reassessments and counseling as needed.
Provides crisis intervention and individual or family counseling when indicated.
Advocates for patient/family needs and coordinates with community services for referrals.
Offers ongoing counseling related to death and dying as needed.
Participates in Bereavement Team activities as assigned.
