Job summary

AdvisaCare Home Health and Hospice seeks a full-time MSW to join our team serving Genesee and Lapeer counties, with Troy as the parent agency. You will provide compassionate social work support to patients and families facing life-limiting illness.

The role includes psychosocial assessments, care planning, bereavement support, and collaboration with the IDT. You will document visits and referrals, coordinate with community resources, and advocate for patient and family needs; a valid driver's

Qualifications

• MSW degree from a CSWE-accredited program.
• One year of home health care experience preferred.
• Hospice experience preferred.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs psychosocial assessments and assists in goal-directed care planning.
• Collaborates with IDT to address psychosocial, emotional, spiritual, and family needs.
• Documents visits, calls, and referrals timely in patient records.
• Provides crisis intervention and bereavement support as needed.
• Develops community referrals and advocates for patient/family needs.
