Job summary

Choice Health at Home is seeking a Social Worker (MSW) to provide compassionate, home-based psychosocial care for patients across ages within the Tucson hospice program. You will assess needs, coordinate services, and support families through end-of-life planning.

As part of the interdisciplinary team, you will conduct assessments within 5 days of hospice election, assist with advance directives, counseling, discharge planning, and ensure patient privacy and respectful care.

Qualifications

• MSW from an accredited school.
• One or more years of Hospice experience.

Responsibilities

• Provide coordinated home-based care for patients of all ages and perform psychosocial assessments and referrals.
• Participate in interdisciplinary team meetings and care planning to coordinate patient care.
• Counsel patients and families on end-of-life, death, and grief.
• Assist with advance directives, discharge planning, and family counseling.
