Job summary

Preferred Hospice of Missouri in Ozark, MO is seeking an experienced Social Worker to join our hospice interdisciplinary team serving the Ozark area. Qualifications

• MSW preferred; CSWE-accredited program.
• BSW considered with relevant experience.

Responsibilities

• Visit patients at home or facilities to perform psychosocial assessments.
• Document in electronic medical record in a timely manner.
• Utilize community resources such as Meals on Wheels.
• Support patients, families, and caregivers with death and dying and grief.
• Educate patients and families about hospice services and admission paperwork.
