Job summary

Day City Hospice is seeking a Medical Social Worker to provide psychosocial support to hospice patients and families in Dayton, Ohio and surrounding areas.

Responsibilities include assessments, care planning, counseling, and coordinating resources while maintaining documentation consistent with individualized plans of care. A Bachelor's in Social Work and Ohio licensure are required, with a Master’s preferred; travel and valid driver’s license are expected.

Qualifications

• Bachelor's degree in Social Work required (Master's degree preferred)
• Current Ohio Social Work Licensure required (LSW, LISW, or LPSW-S)
• Minimum of 1 year of healthcare or hospice experience
• Valid driver's license and reliable transportation

• Conduct timely psychosocial assessments of patients and families/caregivers
• Develop and support individualized care plans based on patient needs
• Provide counseling to patients, families, and staff related to stress and coping challenges
• Maintain accurate and timely documentation in accordance with care plans
• Assist with applications for SSI, Medicaid, Hospice benefits, and waiver programs
• Coordinate community resources including social, health, pharmaceutical, and welfare services
• Serve as a liaison between patients, families, caregivers, and community agencies
• Deliver exceptional service to patients, referral sources, and interdisciplinary team members

Position Summary

Qualifications

Education:

• Bachelor's degree in Social Work required (Master's degree preferred)
• Acceptable alternative degrees: Psychology, Sociology, or related field (must meet supervision and experience requirements)

Licensure:

• Current Ohio Social Work Licensure required (LSW, LISW, or LPSW-S)

Experience:

• Minimum of 1 year of healthcare or hospice experience
• Experience supporting patients and families in a clinical or community-based setting preferred

Other Requirements:

• Valid driver's license and reliable, insured transportation
• Strong interpersonal, communication, and organizational skills
