Job summary

Interim HealthCare seeks a Hospice Medical Social Worker to serve Hampton Roads, VA and surrounding areas. You’ll counsel patients and families, coordinate resources, and support end-of-life planning while collaborating with the interdisciplinary hospice team.

The role emphasizes compassionate communication, advocacy, and ongoing psychosocial support, with opportunities for growth and CEUs in a supportive work environment.

Qualifications

• Master’s in Social Work (CSWE-accredited) required.
• Virginia social work license active and in good standing.
• Minimum 1 year of healthcare experience; hospice preferred.

Responsibilities

• Conduct psychosocial assessments and contribute to the IDG care plan.
• Provide individual and family counseling for emotional needs.
• Offer grief support and connect families with community resources.
• Assist with end-of-life planning and advance directives, with physician collaboration.
• Support discharge planning and ongoing psychosocial follow-up throughout hospice care.
