Job summary

Interim HealthCare Inc. in Hampton Roads, VA seeks a Hospice Medical Social Worker to provide counseling, resource coordination and advocacy to patients and families facing end-of-life challenges.

You will assess psychosocial needs, support care planning with the IDG, and guide families through difficult decisions, while maintaining compassion and professionalism. Competitive benefits offered.

Qualifications

• Master's degree in Social Work from a CSWE-accredited program.
• Active Virginia Social Work license in good standing.
• Minimum 1 year of healthcare experience; hospice experience preferred.
• Strong verbal and written communication skills.
• Ability to work effectively in a team within an IDG framework.

Responsibilities

• Conduct psychosocial assessments of patients and families.
• Provide individual and family counseling for mental and emotional needs.
• Coordinate resources and support for anticipatory and grief needs.
• Assist with advance directives and end-of-life planning.
• Collaborate with physicians and staff on social and emotional factors.
• Support discharge planning and follow-up throughout hospice care.
