Job summary

Interim HealthCare Home Health and Hospice is seeking a Hospice Medical Social Worker to visit patients at home, provide psychosocial support, and help families access resources and counseling to improve quality of life at end of life.

You will work with an interdisciplinary hospice team, assess needs, document progress, counsel families, and advocate for patients. A CSWE-accredited BA in Social Work and state licensure are required; hospice experience is preferred.

Qualifications

• Bachelor’s degree in social work accredited by the CSWE.
• Licensed to practice as a Social Worker in the state of employment; license must be active and in good standing.
• Hospice experience preferred.

Responsibilities

• Provide home-based social services to patients dealing with end-of-life issues.
• Work as part of a hospice team with an interdisciplinary approach.
• Monitor patient’s psychosocial condition and identify social and emotional needs.
• Conduct patient assessments, document progress and ensure progress toward goals.
• Provide counseling, community resource planning, crisis intervention and advocacy.
• Consult with family and caregivers on patient’s plan of care and progression.
