Job summary

Concho Hearts Hospice is seeking a compassionate PRN Hospice CNA/Aide to support patients and families in home or facility settings across Wise and Montague Counties.

You will provide personal care, assist with daily living activities, observe patient changes, document care, and communicate with the hospice team, focusing on comfort, dignity, and meaningful connections for patients at the end of life.

Qualifications

• Active CNA certification (Texas).
• Compassionate, patient-centered approach to care.
• Strong communication and teamwork skills.
• Reliable transportation and willingness to travel locally.

What You'll Do:
