Ascend Hospice in Overland Park, KS is seeking a Social Worker to provide psychosocial support for patients and families facing terminal illness. The role includes assessing emotional factors, coordinating care with community resources, and delivering counseling as needed.

Qualified candidates hold a Master's in Social Work with LMSW license and have experience in hospice settings, strong communication skills, and willingness to travel. This is a full-time opportunity with competitive benefits.

Responsibilities

• Assesses emotional factors related to terminal illness.
• Assesses environmental resources and obstacles to maintain safety.
• Provides emotional support, short-term crisis intervention and individual and/or family counseling when indicated.
• Maintains contact with appropriate community agencies and services to promote interagency cooperation and to facilitate referrals.
• Evaluates patient/family response to psychosocial interventions.
• Assesses special needs related to cultural diversity including communication, space, role of family members and special traditions.
• Performs comprehensive initial psychosocial assessments and assesses the patient/family psychosocial status, risk of suicide and/or abuse or neglect.
