Job summary

Spirit Hospice of Georgia is seeking a licensed Social Worker to join our team dedicated to compassionate, patient-centered care. The role emphasizes supporting patients and families during life’s challenging moments with grace and humor when appropriate.

You will collaborate with nurses, chaplains, aides, doctors and volunteers as part of our interdisciplinary team to develop tailored care plans and provide counseling and resources for families facing grief and transition.

Qualifications

• Current Social Work License in Georgia.
• Hospice, medical, or mental health experience preferred.
• Ability to hold space with empathy, resilience and humor.
• Strong communication, organizational and advocacy skills.

Responsibilities

• Walk with patients and families through meaningful moments.
• Help families navigate grief, fear, and paperwork.
• Lead with compassion and advocate for patients.
• Collaborate with the interdisciplinary team (IDT) including nurses, chaplains, aides, doctors and volunteers.

What you’ll do:

• Walk alongside patients and families through some of life’s hardest but most meaningful moments.
• Help families navigate the messy parts.. grief, fear, and paperwork.
• Lead with compassion, advocate fiercely, and remind people that even in loss there’s still love and dignity.
• Collaborate closely with nurses, chaplains, aides, doctors and volunteers in our interdisciplinary team (IDT), ensuring care is heart-centered.
• Develop and execute patient-specific care plans
• Offer resources, counseling, and that grounding presence people remember long after the goodbye.

What you bring:

• A current Social Work License in the state of Georgia.
• Hospice, medical, or mental health experience. (and if not hospice yet, a calling to it)
• A steady heart that can hold both tears and laughter in the same breath.
• Strong communication, organizational, and advocacy skills.
